Please use the following guidelines to choose the correct service request form:

• To request research design, methodological or collaboration services use the Tufts CTSI Service Request Form.

• To request clinical nursing or research laboratory services of the Clinical and Translational Research Center (including Core Lab) use the Clinical and Translational Research Center and Core Laboratory Application Form.

It is important to take note of the following:

Services to Investigators

All services provided by Tufts CTSI should be directly provided to investigators or on their behalf.  Therefore, the investigator's name should be included in Investigator Information.  If you are completing this request on behalf of the investigator, your contact information will be requested at the end of the form.

eRA Commons Username

An eRA Commons username is required of all investigators using services of Tufts CTSI for Federal reporting.  If you do not have one please see your research administrator to facilitate this.

Cite Tufts CTSI

The National Institutes of Health (NIH) requires investigators to cite the Tufts CTSI grant if they:

• Used any Tufts CTSI services to support their research
• Received funding through Tufts CTSI
